Submit Residency & Income Verification Documents
Income/public benefits documents and proof of residency are state requirements to be collected prior to enrollment. See below for acceptable documents. Pictures of these documents are accepted.
INCOME: Documentation may include two most recent pay stubs, most recent tax return, most recent W-2, or a verification letter from employer.
RESIDENCY: Documentation should be a monthly utility bill such as a water bill, electric bill, or trash bill. Cell phone bills, insurance statements, and voting cards are not accepted.
There are 3 options to submit your documentation. It is recommended to submit documentation prior to your appointment if you are able.
1. Submit via Google Form (Gmail address required)
2. Email documents to prekscreening@wcusd5.net. Please ensure you include your first/last name as well as your child’s first/last name.
3. Bring these documents with you to your appointment.
